Job Description

About the role

The Brighter Futures Program (knows in sector as Extended Post Care Service) supports all young people existing the child protection care system at the age of 18 across the Brisbane region. The Brighter Futures program provides one-to-one intensive case managed support through an assertive outreach model. The program aims to build the independent living of young people who often have little support with complex backgrounds.

Duties

  • Lead the weekly, fortnightly, monthly, and quarterly analysis of client and program impact trends, including individual staff performance. Ensure data accuracy by completing spot-audits in the case management system, cross referencing case notes, assessments, and case plans against data.
  • Lead the active implementation and driving of client impact improvements with individual staff and team. Deliver targeted in-field coaching to improve individual staff performance. Provide team clarity on core focus and how to achieve impact.
  • Complete a weekly, monthly, and quarterly update of the ‘Staff Workflow Data’ outlining each client in the program and their progress against each phase of work in the model of care/staff practice workflow process.
  • Lead the direct support and role clarity guidance to improve individual staff adherence to the consistent delivery of the model of care. Provide structured coaching, efficiency planning, and motivation of staff.
  • Ensure supervision is largely focused on client impact and quality in the model of care interventions.
  • Provide ad-hoc debriefing and guidance to staff, ensuring a proactive response in supporting front-line staff and client crisis episodes.   
  •  Ensure all eligible clients (Indigenous) complete a 715 Health Assessment at their local ATSICHS medical clinic, or preferred provider, prior to case closure. Ensure no case closure is approved until completed.   
  • Lead the management and direct delivery of the new Community Services orientation approach for all new staff across the branch, working closely with Managers/Team Leaders for each program.

Desired Skills and Experience

COVID-19 Requirement

  • Immunisation history showing you have received two doses of a Covid - 19 vaccine is encouraged.

Required

 

  • Bachelor’s degree qualification in Social Work/ Human Services/ Psychology/ Sociology/ Criminology/ Counselling or relevant field and minimum three (3) years’ experience in a front-line case management practitioner position in similar field e.g. Child Protection, Family Support, Youth Support, Youth Justice, Housing, Domestic Violence etc.

OR

  • Diploma qualification in Community Services/ Child and Family Studies/ Youth Intervention/ Social Work/ Human Services/ Psychology/ Sociology/ Criminology/ Counselling or relevant field and minimum five (5) years’ experience in a front-line case management practitioner position in similar field e.g. Child Protection, Family Support, Youth Support, Youth Justice, Housing, Domestic Violence etc.

AND

  • Minimum 12-months demonstrated experience in managing a team in a Team Leader, Team Coordinator, Manager or similar role and/or in an acting role.

Additional Requirements

  • Strong knowledge of youth services and coordinated supports available across the Brisbane region.
  • Strong knowledge of the child protection Out of Home Care (OOHC) system.
  • Strong knowledge of social service practice interventions and theoretical practice frameworks.
  • Requirement to travel across the Logan and/or Brisbane region, as per program and young person location.
  • Current drivers’ licence.
  • Flexibility to work outside 8:30am-5pm business hours in meeting individual client needs and provide responsive care.
  • Working with Children’s Card (Blue Card) or be eligible to apply.
  • Valid police check.
