Job Description

About the role

The Housing and Wellbeing Intake Practitioner will work with individuals and families in partnership to empower and build their agency to make effective change and overcome their identified housing and wellbeing needs. The role is heavily outreach based, engaging individuals and families in crisis accommodation, street outreach and refuges, and translating initial engagement into a strong ongoing partnership.

Duties

  • Provide supports and interventions, based on the program model of care.
  • Provide intent-based practice with clients, utilizing the Human Service Practice Framework as the core basis of requirements when work with clients across each stage of support, following the guidelines and required tools.
  • Provide high quality intake practices and record keeping that adhere to operational requirements.
  • Exhibit proactive and welcoming interaction via assertive outreach methods to effectively involve clients in our services.
  • Integrate crisis management principles and effectively address immediate needs, mitigate the impact of crises, and empower clients to overcome challenges and achieve long-term stability and resilience.
  • Demonstrate warm and positive engagement to clients both when commencing tenancy and vacating.
  • Work to foster productive relationships with all staff internally and externally.

  • Permanent full time position with starting annual salary of $83,500 (depending on qualifications and experience).

Desired Skills and Experience

COVID-19 Requirement

  • Immunisation history showing  you have received two doses of a Covid -19 vaccine is encouraged.

Required

  • Current Bachelor’s Degree, in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention or Criminology with 2 years’ relevant experience in social services field

OR

  • Current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, Child and Family Intervention with 3 years’ relevant experience in social service field

AND

With at least 1 year of abovementioned experience in a case management role.

  • Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets.
  • Demonstrated ability to build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports.
  • Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.
  • Demonstrated ability to work with the whole-of-family, irrespective of primary client, in meeting the totality of family needs.
  • Demonstrated knowledge and understanding of the factors contributing to vulnerability, disengagement, and intergenerational trauma of client group.

Additional requirements

  • Requirement to travel across the Logan and/or Brisbane region, as per program location.
  • Current drivers’ licence.
  • Flexibility to work outside 8:30am-5pm business hours in meeting individual client needs and provide responsive care.
  • Working with Children’s Card (Blue Card) or be eligible to apply.
  • Valid police check.
